Xeljanz and Xeljanz XR increase the risk of severe cardiovascular events according to an FDA study. Heart attacks, strokes, cancer, blood clots, and death are all possibilities. This clinical investigation was to evaluate the adverse effects of Xeljanz and Xeljanz XR on another rheumatoid arthritis medicine known as tumor necrosis factor (TNF) blockers. Lower Xeljanz dosages were connected to an increased risk of death and blood clots. This contrasts with prior findings that showed severe diseases and death were only linked to larger doses of the medicine. For Xeljanz, the FDA ordered a boxed warning, the agency’s most visible caution, cautioning of the risk for fatal outcomes. 

The FDA also issued revised warnings for two additional JAK inhibitors in the same class as Xeljanz. While these have not been tested in large-scale studies, these drugs, Rinvoq and Olumiant, may have similar health risks as Xeljanz. Patients are cautioned that because these JAK inhibitors function in the same way as Xeljanz, they may have an elevated risk of significant cardiovascular problems.
